| Fees: Patients who don’t have Medicaid have to pay for sports ($50) and college ($56-65) physicals. There are also fees for immunizations that are not covered by Vaccines for Children. |
| Parent Permission: Parents need to come in and sign for sports/college physicals and regular (not sex-related) immunizations. Teens who say they’ve been sexually active don’t need parental permission for care. |
| Confidentiality: Confidential but need parents present for sports/college physicals and some immunizations. |
Documentation: The clinic will see teens anonymously, but it would be helpful to come in with your Medicaid card, immunization records and social security number. |
